Please submit your duly signed Quotation Form/Financial Offer, including the Terms and Conditions, together with the following documentary requirements on or before October 13, 2026 at the Municipal General Services Office, Brgy. Roxas, Solano, Nueva Vizcaya : PhilGEPS Certificate of Registration; Mayor’s/Business Permit; and N otarized Omnibus Sworn and if applicable, Original Notarized Secretary’s Certificate in case of a corporation, partnership, or cooperative; or Original Special Power of Attorney of all members of the joint venture giving full power and authority to its officer to sign the OSS and do acts to represent the Bidder. Interested suppliers are encouraged to use the Quotation Form provided to minimize errors or omissions of the required provisions. Quotations, including the required documentary requirements, received after the deadline shall not be accepted. The Head of the Procuring Entity (HoPE) reserves the right to reject any and all quotations, declare a failure of procurement, or not award the contract in accordance with the applicable provisions of RA No. 12009 and its IRR, without thereby incurring any liability to the affected supplier or suppliers.
