Το τελευταίο κωδικοποιημένο καταστατικό της εταιρείας, όπως έχει καταχωρηθεί στο ΓΕΜΗ.\n7. Απόσπασμα ποινικού μητρώου των διαχειριστών της εταιρείας σε περίπτωση Ο.Ε., Ε.Ε.,\n Ι.Κ.Ε. και Ε.Π.Ε. ή σε περίπτωση Α.Ε όλων των μελών του Δ.Σ το οποίο θα πρέπει να έχει\n εκδοθεί έως τρείς (3) μήνες πριν την υποβολή του.\n8. Βεβαίωση εγγραφής και καταβολής ετήσιας συνδρομής στο αρμόδιο βιοτεχνικό ή εμπορικό\n ή βιομηχανικό ή επαγγελματικό επιμελητήριο, οι οποίες θα πρέπει να έχουν εκδοθεί έως\n τριάντα (30) εργάσιμες ημέρες πριν την υποβολή τους.\n\nΔιευκρινίζεται ότι:\nΣε περίπτωση χρήσης υπεργολάβου από τον οικονομικό φορέα για τη εκτέλεση της σύμβασης,\nθα πρέπει να αναφερθεί στην υποβαλλόμενη προσφορά το τμήμα της σύμβασης που\nπροτίθεται να ανατεθεί υπό μορφή υπεργολαβίας σε αυτόν, καθώς και τα στοιχεία του\nυπεργολάβου.
