Α. 1,00 1.000,00\n\nΣημειώνεται ότι προβλέπεται δικαστική προστασία ενώπιον του Διοικητικού Εφετείου της έδρας της αναθέτουσας αρχής, κατά τα οριζόμενα στα άρθρα 45 έως 56 του π.δ. 18/1989 (Α’ 8), το οποίο αποφαίνεται αμετακλήτως, χωρίς να επιτρέπεται η προηγούμενη άσκηση άλλης ειδικής ή ενδικοφανούς διοικητικής προσφυγής. Το παράβολο για την άσκηση της αίτησης ακύρωσης και της αίτησης αναστολής ορίζεται ίσο με το πέντε τοις εκατό (5%) επί της εκτιμώμενης αξίας της σύμβασης (άρθρο 127 παρ.1 του Ν.4412/2016, όπως τροποποιήθηκε από το άρθρο 55 του Ν.4782/2021).\n\nΠρος απόδειξη της μη συνδρομής των λόγων αποκλεισμού ο προσφέρων προσκομίζει τα παρακάτω δικαιολογητικά:\n1. α) Υπεύθυνη δήλωση εκ μέρους του οικονομικού φορέα, σε περίπτωση φυσικού προσώπου ότι δεν συντρέχουν οι λόγοι αποκλεισμού της παραγράφου 1 του άρθρου 73 του Ν.4412/2016 ή β) απόσπασμα ποινικού μητρώου.\n2. Ασφαλιστική ενημερότητα (άρθρο 80 παρ.2 του Ν.4412/2016), συμμετοχής σε διαγωνισμό με ισχύ και κατά το χρόνο υποβολής της προσφοράς.\n3. Φορολογική ενημερότητα για κάθε νόμιμη χρήση εκτός είσπραξης χρημάτων με ισχύ και κατά το χρόνο υποβολής της προσφοράς.\n4. Πιστοποιητικό Γ.Ε.Μ.Η από το οικείο επιμελητήριο.\n5.
