ΦΙΛΙΠΠΑΙΩΝ (ΟΜΑΓΑ 02)» πξνυπνινγηζκνχ 2.966,08€\nκε ΦΠΑ 24%\n\nΟ αλάδνρνο ππνρξενχηαη, γηα ηελ πιεξσκή ηνπ, λα ππνβάιιεη ειεθηξνληθφ ηηκνιφγην ζχκθσλα κε\nηηο δηαηάμεηο ησλ άξζξσλ 148 έσο 154 ηνπ λ.4601/2019 (ΦΔΚ 44/2019 ηεχρνο Α’), ηεο ΚΥΑ 98979\nΔΞ 2021/10.08.2021 (ΦΔΚ 3766/2021 ηεχρνο B’), ηεο ΚΥΑ 63446/31.05.2021 (ΦΔΚ 2338/2021\nηεχρνο Β') θαη ηεο ΚΥΑ 52445 ΔΞ 2023/2023 (ΦΔΚ 2385/2023 ηεχρνο Β').\n\nΟ Κφδηθός ηες Αλαζέηοσζας Αρτής γηα ηελ Ηιεθηροληθή Σηκοιόγεζε, ζηο Μεηρώο\nΑλαζεηοσζώλ Αρτώλ Ηιεθηροληθής Σηκοιόγεζες είλαη 1007.Δ81501.0001\n\n        Γηα ηελ θάιπςε ηεο δαπάλεο ηεο αλσηέξσ σπερεζίας έρεη εθδνζεί α) ε σπ’ αρηζ. 564/2026\nαπφθαζε αλάιεςεο ππνρξέσζεο θαη β) ε βεβαίσζε ηνπ Πξντζηακέλνπ ηεο Οηθνλνκηθήο Υπεξεζίαο,\nεπί ηεο αλσηέξσ απφθαζεο αλάιεςεο ππνρξέσζεο, γηα ηελ χπαξμε δηαζέζηκνπ πνζνχ, ηε ζπλδξνκή\nησλ πξνυπνζέζεσλ ηεο παξ. 1α ηνπ άξζξνπ 4 ηνπ ΠΓ 80/2016 θαη ηε δέζκεπζε ζην νηθείν Μεηξψν\nΓεζκεχζεσλ ηεο αληίζηνηρεο πίζησζεο.\n\n       Η παξάδνζε ηεο πξνζθνξάο ζα γίλεη εληφο δέθα (10) εκεξψλ ζην γξαθείν Πξσηνθφιινπ ζε\nζθξαγηζκέλν θάθειν κε ηίηιν Οηθολοκηθή Προζθορά γηα ηελ σπερεζία «ΜΙ΢ΘΩ΢Η\nΜΗΥΑΝΗΜΑΣΩΝ ΓΙΑ ΢ΤΝΣΗΡΗ΢Η ΣΟΤ ΑΓΡΟΣΙΚΟΤ ΟΓΙΚΟΤ ΓΙΚΣΤΟΤ Γ.Δ. Τποθάθειος δηθαηοιογεηηθώλ\n1.Ποηληθό κεηρώο ή ελαιιαθηηθά Τπεύζσλε δήιφζε (άξζξν 80 παξ. 9 ηνπ Ν.4412/2016, φπσο\nζπκπιεξψζεθε κε ηελ παξ. 7αγ ηνπ άξζξνπ 43 ηνπ Ν.4506/2019) εθ κέξνπο ηνπ νηθνλνκηθνχ θνξέα,\nζε πεξίπησζε θπζηθνχ πξνζψπνπ φηη δελ ζπληξέρνπλ νη ιφγνη απνθιεηζκνχ ηεο παξαγξάθνπ 1 ηνπ\nάξζξνπ 73 ηνπ Ν.4412/2016. Σε πεξίπησζε λνκηθνχ πξνζψπνπ ε πξναλαθεξφκελε ππεχζπλε\nδήισζε ππνβάιιεηαη εθ κέξνπο ηνπ λφκηκνπ εθπξνζψπνπ, φπσο απηφο νξίδεηαη ζην άξζξν 79Α ηνπ\nΝ.4412/2016.\n\nΣε πεξίπησζε λνκηθνχ πξνζψπνπ ε πξναλαθεξφκελε ππεχζπλε δήισζε ππνβάιιεηαη εθ κέξνπο ηνπ\nλφκηκνπ εθπξνζψπνπ, φπσο απηφο νξίδεηαη ζην άξζξν 79 Α ηνπ Ν. 4412/2016, φπσο απηφ\nηξνπνπνηήζεθε κε ην άξζξν 28 ηνπ Ν. 4782/2021. Η ππνρξέσζε απνθιεηζκνχ νηθνλνκηθνχ θνξέα\nεθαξκφδεηαη επίζεο φηαλ ην πξφζσπν εηο βάξνο ηνπ νπνίνπ εθδφζεθε ακεηάθιεηε θαηαδηθαζηηθή\nαπφθαζε είλαη κέινο ηνπ δηνηθεηηθνχ, δηεπζπληηθνχ ή επνπηηθνχ νξγάλνπ ηνπ ελ ιφγσ νηθνλνκηθνχ\nθνξέα ή έρεη εμνπζία εθπξνζψπεζεο, ιήςεο απνθάζεσλ ή ειέγρνπ ζε απηφ. Η ππνρξέσζε ηνπ\nπξνεγνχκελνπ εδαθίνπ αθνξά :\nα) ζηηο πεξηπηψζεηο εηαηξεηψλ πεξηνξηζκέλεο επζχλεο (Δ.Π.Δ.), ηδησηηθψλ θεθαιαηνπρηθψλ\nεηαηξεηψλ (Ι.Κ.Δ.) θαη πξνζσπηθψλ εηαηξεηψλ (Ο.Δ. θαη Δ.Δ.), ηνπο δηαρεηξηζηέο, ή\nβ) ζηηο πεξηπηψζεηο αλσλχκσλ εηαηξεηψλ (Α.Δ.), ηνλ δηεπζχλνληα ζχκβνπιν, ηα κέιε ηνπ\nΓηνηθεηηθνχ Σπκβνπιίνπ, θαζψο θαη ηα πξφζσπα ζηα νπνία κε απφθαζε ηνπ Γηνηθεηηθνχ\nΣπκβνπιίνπ έρεη αλαηεζεί ην ζχλνιν ηεο δηαρείξηζεο θαη εθπξνζψπεζεο ηεο εηαηξείαο, ή\nγ) ζηηο πεξηπηψζεηο ησλ ζπλεηαηξηζκψλ, ηα κέιε ηνπ Γηνηθεηηθνχ Σπκβνπιίνπ, ή\nδ) ζηηο ππφινηπεο πεξηπηψζεηο λνκηθψλ πξνζψπσλ, ηνλ θαηά πεξίπησζε λφκηκν εθπξφζσπν (άξζξν\n80 παξ. 9 ηνπ Ν. 4412/2016, φπσο απηφ ηξνπνπνηήζεθε κε ην άξζξν 29 ηνπ Ν. 4782/2021 θαη άξζξν\n73 παξ.1 ηνπ Ν. 4412/2016, φπσο απηφ αληηθαηαζηάζεθε απφ ην άξζξν 22 ηνπ Ν. 4782/2021).\n\n2. Τπεύζσλε δήιφζε, κέζσ ηεο εληαίαο ςεθηαθήο πχιεο www.gov.gr, αλά πεξίπησζε, λφκηκνπ\nεθπξνζψπνπ ηνπ λνκηθνχ πξνζψπνπ/ νληφηεηαο, ζηελ νπνία δειψλεη φηη ην λνκηθφ πξφζσπν/\nνληφηεηα, ην νπνίν εθπξνζσπεί λφκηκα, δελ έρεη θαηαδηθαζηεί ακεηάθιεηα γηα θαλέλα απφ ηα\nαδηθήκαηα δσξνδνθίαο ηνπ άξζξνπ 73 παξ. 1 ηνπ λ. 4412/2016, θαη’ εθαξκνγή ησλ δηαηάμεσλ ησλ\nάξζξσλ 134- 135 ηνπ λ. 5090/2024.\n\n3. Αποδεηθηηθό ελεκερόηεηας γηα τρέε προς ηο Γεκόζηο (θοροιογηθή ελεκερόηεηα).\nΠηζηνπνηεηηθφ ηεο αξκφδηαο θαηά πεξίπησζε αξρήο, απφ ην νπνίν λα πξνθχπηεη πσο είλαη ν\nνηθνλνκηθφο θνξέαο είλαη ελήκεξνο σο πξνο ηηο θνξνινγηθέο ππνρξεψζεηο ηνπ. Τν πηζηνπνηεηηθφ ζα\nπξέπεη λα είλαη ζε ηζρχ θαηά ηελ εκεξνκελία ηεο ππνβνιήο ηεο πξνζθνξάο θαη ζα θέξεη έλδεημε\n«Γηα θάζε λφκηκε ρξήζε εθηφο είζπξαμεο …».\n\n4. Βεβαίφζε αζθαιηζηηθής ελεκερόηεηας.\nΠηζηνπνηεηηθά φισλ ησλ νξγαληζκψλ θνηλσληθήο αζθάιηζεο (αζθαιηζηηθέο ελεκεξφηεηεο) απφ ηα\nνπνία λα πξνθχπηεη φηη ν νηθνλνκηθφο θνξέαο είλαη ελήκεξνο σο πξνο ηηο ππνρξεψζεηο ηνπ πνπ\n\f                   26PROC019989160 2026-10-14\n\nαθνξνχλ ηελ θαηαβνιή ησλ εηζθνξψλ θνηλσληθήο αζθάιηζεο ζε απηνχο ηνπο νξγαληζκνχο. Όιεο νη\nβεβαηψζεηο αζθαιηζηηθήο ελεκεξφηεηαο ζα πξέπεη λα είλαη ζε ηζρχ θαηά ηελ εκεξνκελία ηεο\nππνβνιήο ηεο πξνζθνξάο θαη ζα θέξνπλ έλδεημε «… ζπκκεηνρή ζε δηαγσληζκφ….».\n\n5. Γεληθό Πηζηοποηεηηθό θαη Πηζηοποηεηηθό Τπερεζίας Γ.Δ.ΜΗ. Ιζτύοσζας Δθπροζώπεζες.\nΓηα ηελ απφδεημε ηεο λφκηκεο εθπξνζψπεζεο, ζηηο πεξηπηψζεηο πνπ ν νηθνλνκηθφο θνξέαο είλαη\nλνκηθφ πξφζσπν θαη εγγξάθεηαη ππνρξεσηηθά ή πξναηξεηηθά, θαηά ηελ θείκελε λνκνζεζία, θαη\nδειψλεη ηελ εθπξνζψπεζε θαη ηηο κεηαβνιέο ηεο ζε αξκφδηα αξρή (πρ ΓΔΜΗ), πξνζθνκίδεη ζρεηηθφ\nπηζηνπνηεηηθφ ηζρχνπζαο εθπξνζψπεζεο, Γεληθφ Πηζηνπνηεηηθφ Μεηαβνιψλ θαζψο θαη ην\nθαηαζηαηηθφ ηεο εηαηξείαο ζπλνδεπφκελν απφ φιεο ηηο ηξνπνπνηήζεηο απηνχ. Σε πεξίπησζε λνκηθνχ πξνζψπνπ ε πξναλαθεξφκελε ππεχζπλε\nδήισζε ππνβάιιεηαη εθ κέξνπο ηνπ λφκηκνπ εθπξνζψπνπ, φπσο απηφο νξίδεηαη ζην άξζξν 79Α ηνπ\nΝ.4412/2016.\n\nΣε πεξίπησζε λνκηθνχ πξνζψπνπ ε πξναλαθεξφκελε ππεχζπλε δήισζε ππνβάιιεηαη εθ κέξνπο ηνπ\nλφκηκνπ εθπξνζψπνπ, φπσο απηφο νξίδεηαη ζην άξζξν 79 Α ηνπ Ν. 4412/2016, φπσο απηφ\nηξνπνπνηήζεθε κε ην άξζξν 28 ηνπ Ν. 4782/2021. Η ππνρξέσζε απνθιεηζκνχ νηθνλνκηθνχ θνξέα\nεθαξκφδεηαη επίζεο φηαλ ην πξφζσπν εηο βάξνο ηνπ νπνίνπ εθδφζεθε ακεηάθιεηε θαηαδηθαζηηθή\nαπφθαζε είλαη κέινο ηνπ δηνηθεηηθνχ, δηεπζπληηθνχ ή επνπηηθνχ νξγάλνπ ηνπ ελ ιφγσ νηθνλνκηθνχ\nθνξέα ή έρεη εμνπζία εθπξνζψπεζεο, ιήςεο απνθάζεσλ ή ειέγρνπ ζε απηφ. Η ππνρξέσζε ηνπ\nπξνεγνχκελνπ εδαθίνπ αθνξά :\nα) ζηηο πεξηπηψζεηο εηαηξεηψλ πεξηνξηζκέλεο επζχλεο (Δ.Π.Δ.), ηδησηηθψλ θεθαιαηνπρηθψλ\nεηαηξεηψλ (Ι.Κ.Δ.) θαη πξνζσπηθψλ εηαηξεηψλ (Ο.Δ. θαη Δ.Δ.), ηνπο δηαρεηξηζηέο, ή\nβ) ζηηο πεξηπηψζεηο αλσλχκσλ εηαηξεηψλ (Α.Δ.), ηνλ δηεπζχλνληα ζχκβνπιν, ηα κέιε ηνπ\nΓηνηθεηηθνχ Σπκβνπιίνπ, θαζψο θαη ηα πξφζσπα ζηα νπνία κε απφθαζε ηνπ Γηνηθεηηθνχ\nΣπκβνπιίνπ έρεη αλαηεζεί ην ζχλνιν ηεο δηαρείξηζεο θαη εθπξνζψπεζεο ηεο εηαηξείαο, ή\nγ) ζηηο πεξηπηψζεηο ησλ ζπλεηαηξηζκψλ, ηα κέιε ηνπ Γηνηθεηηθνχ Σπκβνπιίνπ, ή\nδ) ζηηο ππφινηπεο πεξηπηψζεηο λνκηθψλ πξνζψπσλ, ηνλ θαηά πεξίπησζε λφκηκν εθπξφζσπν (άξζξν\n80 παξ. 9 ηνπ Ν. 4412/2016, φπσο απηφ ηξνπνπνηήζεθε κε ην άξζξν 29 ηνπ Ν. 4782/2021 θαη άξζξν\n73 παξ.1 ηνπ Ν. 4412/2016, φπσο απηφ αληηθαηαζηάζεθε απφ ην άξζξν 22 ηνπ Ν. 4782/2021).\n\n2. Τπεύζσλε δήιφζε, κέζσ ηεο εληαίαο ςεθηαθήο πχιεο www.gov.gr, αλά πεξίπησζε, λφκηκνπ\nεθπξνζψπνπ ηνπ λνκηθνχ πξνζψπνπ/ νληφηεηαο, ζηελ νπνία δειψλεη φηη ην λνκηθφ πξφζσπν/\nνληφηεηα, ην νπνίν εθπξνζσπεί λφκηκα, δελ έρεη θαηαδηθαζηεί ακεηάθιεηα γηα θαλέλα απφ ηα\nαδηθήκαηα δσξνδνθίαο ηνπ άξζξνπ 73 παξ. 1 ηνπ λ. 4412/2016, θαη’ εθαξκνγή ησλ δηαηάμεσλ ησλ\nάξζξσλ 134- 135 ηνπ λ. 5090/2024.\n\n3. Αποδεηθηηθό ελεκερόηεηας γηα τρέε προς ηο Γεκόζηο (θοροιογηθή ελεκερόηεηα).\nΠηζηνπνηεηηθφ ηεο αξκφδηαο θαηά πεξίπησζε αξρήο, απφ ην νπνίν λα πξνθχπηεη πσο είλαη ν\nνηθνλνκηθφο θνξέαο είλαη ελήκεξνο σο πξνο ηηο θνξνινγηθέο ππνρξεψζεηο ηνπ. Τν πηζηνπνηεηηθφ ζα\nπξέπεη λα είλαη ζε ηζρχ θαηά ηελ εκεξνκελία ηεο ππνβνιήο ηεο πξνζθνξάο θαη ζα θέξεη έλδεημε\n«Γηα θάζε λφκηκε ρξήζε εθηφο είζπξαμεο …».\n\n4. Υπεύθυνη δήλωση εκ μέρους του οικονομικού φορέα, σε περίπτωση φυσικού\nπροσώπου ότι δεν συντρέχουν οι λόγοι αποκλεισμού της παραγράφου 1 του\nάρθρου      73    του     Ν.4412/2016.      Σε      περίπτωση    νομικού      προσώπου     η\nπροαναφερόμενη      υπεύθυνη     δήλωση       υποβάλλεται       εκ   μέρους   του     νόμιμου\nεκπροσώπου, όπως αυτός ορίζεται στο άρθρο 79Α του Ν.4412/2016.Η υποχρέωση\nαποκλεισμού οικονομικού φορέα εφαρμόζεται επίσης όταν το πρόσωπο εις βάρος\nτου οποίου εκδόθηκε αμετάκλητη καταδικαστική απόφαση είναι μέλος του\nδιοικητικού, διευθυντικού ή εποπτικού οργάνου του εν λόγω οικονομικού φορέα ή\nέχει εξουσία εκπροσώπησης, λήψης αποφάσεων ή ελέγχου σε αυτό.     "text": "ΔΙΑΚΗΡΥΞΗ ΔΙΑΓΩΝΙΣΜΟΥ\nΟ Δήμαρχος Γρεβενών διακηρύσσει τη Δημόσια, Ανοικτή, Ηλεκτρονική διαδικασία επιλογής αναδόχου για την προμήθεια με τίτλο: «Προμήθεια και εγκατάσταση εξοπλισμού για την αναβάθμιση στο ΔΑΚ «ΜΙΛΤΟΣ ΤΕΝΤΟΓΛΟΥ».\nΠΡΟΥΠΟΛΟΓΙΣΜΟΣ: 1.500.000,00€ (συμπεριλαμβανομένου Φ.Π.Α 24% ) που θα διεξαχθεί σύμφωνα με τις διατάξεις του Ν. 4412/16 και τους όρους της παρούσας και καλεί τους ενδιαφερόμενους οικονομικούς φορείς να υποβάλλουν προσφορά για την ανάδειξη αναδόχου της ανωτέρω προμήθειας.\n\n1.3 Συνοπτική Περιγραφή φυσικού και οικονομικού αντικειμένου της σύμβασης\nΑντικείμενο της σύμβασης είναι η προμήθεια και εγκατάσταση τάπητα στίβου τύπου ταρτάν, φυσικού χλοοτάπητα και λοιπού εξοπλισμού στο Δημοτικό Αθλητικό Κέντρο ‘’ΜΙΛΤΟΣ ΤΕΝΤΟΓΛΟΥ’’ του Δήμου Γρεβενών.\nΤα προς προμήθεια είδη κατατάσσονται στους ακόλουθους κωδικούς του Κοινού Λεξιλογίου δημοσίων συμβάσεων (CPV):\n37421000-5 Τάπητες γυμναστήριων\n16150000-1 Κύλινδροι χλοοτάπητα ή αθλητικών γηπέδων\n43323000-3 Εξοπλισμός άρδευσης\n31518600-6 Προβολείς φωτισμού\n34928520-9 Στύλοι φωτισμού\nΗ εκτιμώμενη αξία της σύμβασης ανέρχεται στο ποσό των 1.500.000,00 €, συμπεριλαμβανομένου ΦΠΑ 24%.\nΗ διάρκεια της σύμβασης ορίζεται σε δεκαοχτώ (18) μήνες.\nΑναλυτική περιγραφή του φυσικού και οικονομικού αντικειμένου της σύμβασης δίδεται στο ΠΑΡΑΡΤΗΜΑ Ι ή σε άλλο περιγραφικό έγγραφο της παρούσας διακήρυξης.\nΗ σύμβαση θα ανατεθεί με το κριτήριο της πλέον συμφέρουσας από οικονομική άποψη προσφοράς, βάσει τιμής (χαμηλότερη τιμή) για το σύνολο της προμήθειας.\n\n1.5 Προθεσμία παραλαβής προσφορών\nΗ καταληκτική ημερομηνία παραλαβής των προσφορών είναι η 06/11/2026 και ώρα 15:00 μ.μ.\nΗ διαδικασία θα διενεργηθεί με χρήση του Εθνικού Συστήματος Ηλεκτρονικών Δημόσιων Συμβάσεων (ΕΣΗΔΗΣ) Προμήθειες και Υπηρεσίες του ΟΠΣ ΕΣΗΔΗΣ.\n\n2.2.2 Εγγύηση συμμετοχής\nΓια την έγκυρη συμμετοχή στη διαδικασία σύναψης της παρούσας σύμβασης, κατατίθεται από τους συμμετέχοντες οικονομικούς φορείς εγγυητική επιστολή συμμετοχής ποσού 24.193,55 €.\n\n2.2.5 Οικονομική και χρηματοοικονομική επάρκεια\nΟι οικονομικοί φορείς απαιτείται να διαθέτουν, επί ποινή αποκλεισμού, μέσο γενικό ετήσιο κύκλο εργασιών των τριών (3) ετών (2023, 2024, 2025) τουλάχιστον ίσο ή μεγαλύτερο της εκτιμώμενης αξίας της παρούσας σύμβασης πλέον ΦΠΑ.\n\n2.2.6 Τεχνική και επαγγελματική ικανότητα\nΟι οικονομικοί φορείς απαιτείται, επί ποινή αποκλεισμού, να συμμορφώνονται με τις απαιτήσεις της τεχνικής περιγραφής – τεχνικών προδιαγραφών της σχετικής Μελέτης (ΠΑΡΑΡΤΗΜΑ Ι).\n\n2.2.7 Πρότυπα διασφάλισης ποιότητας και πρότυπα περιβαλλοντικής διαχείρισης\nΟι οικονομικοί φορείς οφείλουν, επί ποινή αποκλεισμού, να συμμορφώνονται με τα Πρότυπα και τις Πιστοποιήσεις περί διασφάλισης ποιότητας, περιβαλλοντικής διαχείρισης, διαχείρισης υγείας και ασφάλειας στην εργασία και κοινωνικής ευθύνης, συμπεριλαμβανομένων πιστοποιητικών ISO 9001:2015, ISO 14001:2015, ISO 45001:2018 και ISO 37001:2017.
