In accordance with the CAV Statement of Work, the contractor is required to accurately report all transactions by the end of the\rfifth regular business day after receipt and the Action Date entered in CAV must be dated to reflect the actual date of physical receipt. The Contractor must obtain final inspection and acceptance by the Government for all assets within the RTATs established in\rthis contract.\rPlease note freight is handled by Navy CAV or Proxy CAV as stated in NAVSUPWSSFA24 or NAVSUPWSSFA25.\rThroughput Constraint: Contractor must provide a throughput Constraint for each NSN(s). Total assets to be repaired monthly after initial delivery commences: _____/month (contractor is to fill in #). A throughput of \"0\" (zero) per month reflects no throughput\rconstraint.\rInduction Expiration Date: 365 days after contract award date. Any asset received after this date in days is not authorized for repair without bi-lateral agreement between the Contractor and NAVSUP WSS Contracting Officer.\rH. IMPORTANT NOTE:\rReconciliation Price Reduction:\rAny asset(s) for which the contractor does not meet the required RTAT will incur a price reduction per unit/per month the contractor is late. However, any delay determined to be excusable (e.g. Government Delay) will not result in a price reduction. If the\rcontractor does not meet the required RTAT due to inexcusable contractor delay, the Contracting Officer will implement the aforementioned price reduction via a \"Reconciliation Modification\" at the end of the contract performance. Price reductions made pursuant to\r this section shall not limit other remedies available to the Government for failure to meet required RTATs, including but not limited to the Government's right to terminate for default.\rIn reference to the above important note, the contractor provides the following price reduction amount per unit/per month the contractor does not meet the RTAT: $___________, up to a maximum of: $__________. (vendor is to fill in amount)\rI. Your quote MUST include the following information for justification purposes:\r- Repair unit price ________________\r- Total price________________\r- Repair Turnaround Time (RTAT) _____________ days\r- New unit price ______________\rNOTE: If you are not quoting the Government's Requested Repair Turnaround Time (RTAT) or sooner, you must provide your company's capacity constraints below:\r___________________________________________________________________________\r___________________________________________________________________________\r___________________________________________________________________________\r- T&E fee/price (if asset is determined BR/BER): If an item is determined BR, and the contractor provides documentation that costs were incurred during evaluation and determination of BR/BER, the contracting officer will negotiate a reduced contract price, not-to\rexceed (NTE) $___________associated with the repair effort to reflect the BR status of the equipment.\r- The T&E fee (if asset is determined BR/BER) was determined based on: ____________________________________________________________________\r____________________________________________________________________\r- Award to CAGE:________\r- Inspection & Acceptance CAGE, if not the same as Award to CAGE: _________\r- Facility/Subcontractor CAGE (where the asset should be shipped to), if not same as Award to CAGE. __________\r- Facility/Subcontractor or Packaging Facility CAGE (where the asset will be shipped from), if not the same as Award to CAGE. ________\r- Cost breakdown requested, including profit rate (if feasible).\r- Quote expiration date _______________________ (MIN of 90 Days is requested).\r- Delivery Vehicle (if Delivery Order requested) (if your company has a current BOA/IDIQ, for example)\rJ. OPTIONS\rPROSPECTIVE OPTION QUANTITY UP TO ___%\rNOTES:\rThe Government intends to add an option quantity to the resultant contract. In reference to the option quantity, The government requests up to a 100% (10EA) option for a period of 365 days. The option grants the Government the unilateral authority to increase\rthis order up to the above quantity within 365 days of the contract award date. This will incorporate FAR Clause 52.217-6 (Option for Increased Quantity) into the award. The Government may increase the quantity of supplies called for in the Schedule at the unit\rprice specified. The Contracting Officer may exercise the option by written notice to the Contractor within the number of days in FAR 52.217-6 herein. Delivery of the added items shall continue at the same rate as the like items called for under the contract,\runless the parties otherwise agree. A modification will be completed at the time the option is exercised and all current contract terms and conditions will apply to the option quantity.\rPlease indicate if the Contracting Officer may exercise the 100% option quantity for the contract.\rYes ____ or No ____\rOr if you request a variation to the option please let me know in your quote.\r\\\r1. COMMERCIAL SPECIFICATIONS, STANDARDS AND DESCRIPTIONS - THESE SPECIFICATIONS, STANDARDS AND DESCRIPTIONS ARE NOT AVAILABLE FROM GOVERNMENT SOURCES. THEY MAY BE OBTAINED FROM THE PUBLISHERS OR SOCIETIES OF THE APPLICABLE DOCUMENTS.\r6.2 In accordance with DoDI 5230.24 all documents and drawings provided by the U.S. Navy to prospective Contractors must include a \"Distribution Statement\" to inform the contractor of the limits of distribution, and the safeguarding of the information\rcontained on those documents and drawings.\r There are 6 (six) separate distribution statement codes used for non-classified documents and drawings. The definition for each is as follows:\rA... approved for public release; distribution is unlimited.\rB... distribution authorized to US Governments agencies only.\rC... distribution authorized to US Government agencies and their contractors.\rD... distribution authorized to DoD and DoD contractors only.\rE... distribution authorized to DoD Components only.\rF... further distribution only as directed by Commander, Naval Sea\r Systems Command, code 09T.\r6.3 Information regarding abbreviations, symbols and codes appearing on DD Form 1423 - The following information is provided to assist in understanding the intent of the requirement to provide a deliverable item to the government. The explanation of\rabbreviation, symbols and codes found in a block follows the block number as they appear on the DD Form 1423.\rBlock A: Is the actual contract line item no.\rBlock B: Is the actual collective physical list of the deliverable item(s)\r which are part of the total requirement of the contract/purchase\r order.\rBlock C: Is the category of data required, TDP is defined\r in MIL-T-31000. TM is defined in Part 1X, Section B\r of DODI 5000.2. NAVSUP-WSS has reasoned that most DD Form 1423's\r included in our solicitations meet the requirement of TDP as\r NAVSUP-WSS does not regularly purchase only Technical Manuals\r without purchasing hardware and related TDP software. Therefore,\r most NAVSUP-WSS DD Form 1423 category code will be TDP.\rBlock D: Is the name of the parent system, next higher assembly, or the item\r being purchased.\rBlock E: Is the purchase request number, request for proposal number,\r invitation for bid number or another number for tracking and\r monitoring purposes.\rBlock F: Is the successful offerors name and or cage code.\rBlock G: Is the name of the individual and or the code/activity of the\r individual who prepared the DD Form 1423 and included the\r requirement in the Technical Data Package (TDP).\rBlock H: Self explanatory.\rBlock I: Is the name and signature of the individual who approved the content\r and the need for inclusion of the DD Form 1423 in the TDP.\rBlock J: Self explanatory.\rBlock 1: Is the Exhibit Line Item Number (ELIN).\rBlock 2: Is the title of the data item cited in Block 4.\rBlock 3: Is the subtitle of the data item cited in Block 4 and is used if\r the title requires clarification.\rBlock 4: Is the actual Data Item Description (DID) number or the actual\r Technical Manual Contract Requirements Number. In the event\r the DD Form 1423 is requiring a technical manual the numbered\r TMCR will be an attachment to the contract/purchase order.\rBlock 5: Is the section and paragraph area, where the requirement\r statement for the DD Form 1423 will be found.\rBlock 6: Is the activity that will inform the contractor of approval,\r conditional approval or disapproval of the deliverable item.\rBlock 7: Is a code which designates authority for inspection and\r acceptance of the deliverable item. The definition of the\r codes is as follows:\r DD Form 250 Code Inspection Acceptance\r SS (1) (2)\r DD (3) (4)\r SD (1) (4)\r DS (3) (5)\r LT (6) (7)\r NO (8) (8)\r XX (9) (9)\r (1) Inspection at source.\r (2) Acceptance at source.\r (3) No inspection performed at source. Final inspection\r performance at destination.\r (4) Acceptance at destination.\r (5) Acceptance at source. Acceptance based on written\r approval from the Contract Officer.\r (6) Letter of transmittal only. LT should not be used when\r inspection is required. The data is sent by the\r contractor directly to the personnel listed in Block 14\r of the DD Form 1423. LT is used when the contracting\r agency does not desire to have a DD Form 250 for each\r and every piece of data developed by the contractor.\r The only other authorized use of LT is the special\r case where the contracting agency does not desire to\r have separate DD Forms 250 but desires to have a\r Government quality assurance representative perform\r inspection. The Government quality assurance\r representative shall be listed on the distribution\r in Block 14 and requested to provide comments via\r the quality assurance letter of inspection.\r Use of the symbol LT is not authorized for data comprising\r final delivery of Technical Data Package of for Technical\r Manuals. (LT may, however, be used for delivery of\r preliminary TDP's or TM's).\r (7) As specified in Block 8 of the DD Form 1423.\r (8) No inspection or acceptance required. No DD Form 250 or\r letter of transmittal required.\r Use of the symbol NO is not authorized for data comprising\r Technical Data Packages or for Technical Manuals.\r (9) Inspection and acceptance requirements specified elsewhere\r in the contract.\rBlock 8: Is the approval code - Items of critical data requiring\r specific advanced written approval prior to distribution of\r the final data item will be identified by placing an \"A\"\r in this field. This data item requires submission of a\r preliminary draft prior to publication of a final document.\r When a preliminary draft is required, Block 16 of the DD\r Form 1423 will show length of time required for Government\r approval/disapproval and subsequent turn-around time for the\r contractor to resubmit the data after Government\r approval/disapproval has been issued. Block 16 will also\r indicate the extent of the approval requirements, e.g.,\r approval of technical content and/or format.\rBlock 9: Is the distribution statement code which explains how the\r Government can circulate the deliverable item. The definition\r of codes A, B, C, D, E, or F is as follows:\r A. Distribution of the item is unrestricted.\r B. Distribution of the item is limited to agencies only.\r C. Distribution of the item is limited to contractors with\r a cage code and have a DD Form 2345 on file with the\r DLA Logistics Information Services (DLIS) Battle Creek,\r Mich. or Government activities.\r D. Distribution of the item is limited to DOD activities\r and DOD contractors only.\r E. Distribution of the item is limited to DOD components\r only.\r F. Distribution of the item is restricted from Foreign\r Nations and Foreign Nationals.\rBlock 10: Is the frequency which the deliverable item is expected to\r be delivered to the Government. The abbreviations and\r their meaning.\r ANNLY Annually\r ASGEN As generated\r ASREQ As required\r BI-MO Every two months\r BI-WE Every two weeks\r DAILY Daily\r DFDEL Deferred Delivery\r MTHLY Monthly\r ONE/R One time with revisions\r QRTLY Quarterly\r SEMIA Every six months\r WEKLY Weekly\r XTIME Number of times to be submitted\r (1time, 2times...9times)\r Use of these codes requires further explanation in Block 16\r to provide the contractor with guidance necessary to accurately\r price the deliverable data item.\r Other abbreviations not appearing on the above list may on\r occasion be used in Block 10 of the DD Form 1423. When other\r abbreviations are used they will be fully explained in Block 16\r of the DD Form 1423.\rBlock 11: Is the last calendar date, expressed in year/month/day\r format, the deliverable item is to be received by the\r requiring office cited in Block 6 of the DD Form 1423\r for an item with a Block 10 entry indication a single\r delivery. If the item is to be submitted multiple\r times, the number stated is the number of calendar days\r after the frequency cited in Block 10 the item is to be\r received by the requiring office cited in Block 6 of the\r DD Form 1423. On occasion the deliverable item will be\r required to be submitted prior to the end of the frequency\r cited in Block 6. In that event the requirement will be\r fully explained in Block 16 of the DD Form 1423.\rBlock 12: Is the date of first submission of the deliverable item to\r the requiring office (Block 4) expressed in\r year/month/day format. The abbreviations and their\r meaning are as follows:\r ASGEN As generated\r ASREQ As required\r DAC Days after contract date\r DFDEL Deferred Delivery\r EOC End of contract\r EOM End of month\r EOQ End of quarter\r Specific instructions for these requirements will be provided in\r Block 16. If the deliverable item is constrained by a specific\r event or milestone the constraint will be fully explained in Block 16\r of the DD Form 1423. \"As generated\", \"As required\", and \"Deferred\r Delivery\" will always be fully explained in Block 16.\rBlock 13: Is the date of subsequent submission of the deliverable item, after\r the initial submission. Subsequent submission is only used to\r indicate the specific time period. The data is required when\r Block 10 of the DD Form 1423 indicates multiple delivery is\r required. This does not apply to resubmission of a deliverable\r item that has been reviewed by the requiring office and determined\r to be only conditionally acceptable or unacceptable.\rBlock 14a: Will contain the activity name(s) where the deliverable item is to\r be sent. If the activity is other than DCMA or NAVSUP-WSS the full\r name, address (including code) will be specified. That specific\r information will be located in Block 16 of the DD Form 1423.\rBlock 14b: Is the number of copies of draft and or final copies to\r be submitted. When final \"Repro\" copies are to be submitted\r Block 16 will clarify the type of Repro copies required.\r (e.g., vellum, negative, etc.)\rBlock 15: Is the total of each type of copies to be submitted as required\r by Block 14.\rBlock 16: Is the block used to provide additional or clarifying information\r Blocks 1 through 15. This block is also the only area used to\r tailor the document listed in Block 4. Only deletions to the\r minimum requirements stated in the document in Block 4 are\r allowed. Block 16 may also be used to specify the medium for\r delivery of the data.\rBlock 17: Is the block where the bidder or offeror is to enter the\r appropriate price group. The price groups are defined as follows:\r A. Group I - Data which the contractor prepares to satisfy the Government's requirements. The contractor does not need this\r type of data to perform the rest of the contract. Price\r would be based on identifiable direct costs, overhead, General\r and Administrative (G&A) and profit.\r B. Group II - Data essential to contract performance which must be\r reworked or amended to conform to Government requirements.\r The price for data in this group would be based on the direct\r cost to convert the original data to meet Government needs and\r to deliver it, plus allocable overhead, G&A and profit.\r C. Group III - Data which the contractor must develop for his own\r use and which requires no substantial change to conform to\r Government requirements regarding depth of content, format,\r frequency of submittal, preparation, and quality of data. Only\r the costs of reproducing, handling and delivery, plus overhead,\r G&A and profit, are considered in pricing data in this group.\r D. Group IV - Data which the contractor has developed as part\r of his commercial business. Not much of this data is required\r and the cost is insignificant. The item should normally\r be coded \"no charge.\" An example is a brochure or brief manual\r developed for commercial application which will be acquired\r in small quantities, and the added cost is too small to justify\r the expense of computing the charge that otherwise would go\r with the acquisition.\rBlock 18: Enter the total estimated price equal to that portion of the total\r price which is estimated to be attributable to the production\r or development for the government of that ITEM OF DATA. THE\r ENTRY \"N/C\" FOR \"NO CHARGE\" is acceptable.",
