Zamawiający może unieważnić postępowanie o udzielenie zamówienia przed upływem terminu składania ofert, jeżeli wystąpiły okoliczności powodujące, że dalsze prowadzenie postępowania jest nieuzasadnione. IV. Wykonawca składa wraz z ofertą: 1. Oświadczenie składane na podstawie ustawy z dnia 13 kwietnia 2022 – o szczególnych rozwiązaniach w zakresie przeciwdziałania wspieraniu agresji na Ukrainę oraz służących ochronie bezpieczeństwa narodowego wraz z oświadczeniem składanym na podstawie rozporządzenia (UE) nr 833/2014 dotyczącego środków ograniczających w związku z działaniami Rosji destabilizującymi sytuację na Ukrainie (wzór stanowi załącznik nr 1 do SWZ). 2. JEDNOLITY EUROPEJSKI DOKUMENT ZAMÓWIENIA 3. Formularz ofertowy (generowany na platformie zakupowej). W formularzu Wykonawca składa oświadczenie, że oferowane świadczenia nie obejmują produktów ICT, usług ICT lub procesów ICT wskazanych w rekomendacji, o której mowa w art. 33 ust. 4 ustawy z dnia 5 lipca 2018 r. o krajowym systemie cyberbezpieczeństwa, stwierdzającej ich negatywny wpływ na podstawowy interes bezpieczeństwa państw oraz produktu ICT, którego typ został określony w decyzji w sprawie uznania dostawcy za dostawcę wysokiego ryzyka, o której mowa w art. 67b ust. 15 ustawy o krajowym systemie cyberbezpieczeństwa, usług ICT lub procesów ICT określonych w tej decyzji. V. W celu wykazania braku podstaw do wykluczenia z postępowania o udzielenie zamówienia Zamawiający będzie żądał od wykonawcy, który złożył najkorzystniejszą ofertę dokumentów określonych w rozdz. XI SWZ (dotyczy wykonawców oraz odpowiednio wykonawców występujących wspólnie, podmiotów trzecich), w tym: odpisu z właściwego rejestru lub z Centralnej Ewidencji i Informacji o Działalności Gospodarczej, zaświadczenia właściwego naczelnika urzędu skarbowego potwierdzającego, że Wykonawca nie zalega z opłacaniem podatków, zaświadczenia właściwej terenowej jednostki organizacyjnej Zakładu Ubezpieczeń Społecznych lub Kasy Rolniczego Ubezpieczenia Społecznego albo albo innego dokumentu potwierdzającego, że Wykonawca nie zalega z opłacaniem składek na ubezpieczenia społeczne lub zdrowotne; informacji z Krajowego Rejestru Karnego, oświadczenia Wykonawcy, w zakresie art. 108 ust. 1 pkt 5 ustawy, o braku przynależności do tej samej grupy kapitałowej, oświadczenia Wykonawcy o aktualności informacji zawartych w oświadczeniu, o którym mowa w art. 125 ust. 1 ustawy, w zakresie podstaw wykluczenia z postępowania wskazanych przez Zamawiającego, a w celu potwierdzenia spełnienia warunków udziału określonych w Rozdziale VIII ust. 1 SWZ wykazu wykonanych dostaw wraz z referencjami.
